What is it? - It's a simple concept which can make all the difference to the estimated 106,000 Carers who live and use your service in Surrey. The new National Carers Strategy includes an additional £400 million over the next 4 years to provide breaks for Carers. This will represent £1.3 million per year in Surrey. This is a pilot scheme and aims to provide flexible breaks / respite to eligible Carers who are providing care and registered with a GP Practice in Surrey. This will provide registered GP Practices the opportunity to provide an additional service to your patients who are Carers and are eligible to use the service.
How does it Work? - Again its simple. Everything is done on line so it is efficient and speedy. Above all we use secure forms so all the data which is sent is safe. Here's the process:
1) First of all you or one of your team needs to download the supporting material which tells you how the system works and the criteria which is used. You can do this by clicking here (pdf document)
2) Register your practice to use the scheme via our secure on line form by clicking here. You will then be sent a notification when this form is submitted with a unique reference number on it and another once your GP Practice has been registered on the system.
3) Once your registered you are ready to go! If someone comes into your surgery who meets the eligibility criteria outlined in the supporting material you have downloaded, and you feel clinically they would benefit from this scheme simply fill in the short referral form by clicking here. This form is secure and by pressing the submit button will be sent to Surrey Independent Living Council (SILC) who will do all the administration to meet the referral. You will then get a confirmation that the form has been sent with a unique number on it in case you need to follow the referral up with SILC. Once SILC have processed the form and arranged the payment to the Carer you will get an email informing you of this. This can then be put onto the patient/ Carer records. So as you can see it is one simple form to fill in on line.

NHS Surrey have agreed funding of £300,000 for the pilot scheme. This will provide an estimated 500 Carer breaks. During the pilot a monthly monitoring report of spemd will be circulated to all participating practices. This is a pilot project and is resource limited. An evaluation will take place spring 2012 where you will be invited to feedback on the model
